PAYMENT POLICY

Medical services are provided and billed by Flagler Diagnostic and Sleep Disorder Center. Our Facility and Medical Director who reads your study participate in most all of the major health insurance plans in the region. Following are the health plans which we currently accept. If your plan is not listed, contact us and we will try to help, get authorization or provide you with a sleep center that accepts your insurance. However, we can provide a fee for service and you can then submit a claim to your insurance on your own so that you can submit to your insurance company to instruct them to send reimbursements directly to you.

 

Please be aware that with any health plans, there may be deductibles and co-payments for which you could be responsible. All co-pays are due at time of appointment. We require 24 hour notice of a cancellation or you may be subject to a $150.00 cancellation fee.

 

We accept VISA, MASTERCARD and DISCOVER Effective 6/1/2016 2.5% convenience fee will added to CC transactions of $25.00 or more.  We also accept cash and Checks payable to Flagler Diagnostic.

QUESTIONS ABOUT YOUR BILL

What billing or insurance information will I receive?

Once the insurance has paid its portion of the procedure(s) they will mail you an Explanation Of Benefits (EOB) that will state the date of service, procedure, portion you paid, portion the insurance paid and if there is any amount left to be paid or refunded.

How long will it take to get things settled with the insurance company?

Generally, it takes from 45 to 60 days to obtain payment from an insurance carrier.​ However it can take as long as 6 months.

​Why am I getting bills from your Facility when I have Insurance it was authorized?

​Sometimes insurance companies with give a prior authorization for the service however they will always state that it is not a "guarantee of payment". Which means they are okaying the test but they still might not cover the test if they don't feel its medically needed after the test is preformed. Or you have a deductible. We try to get all the bases covered to know exactly how much will be cover, what any out of pocket expense is but Insurance companies, and plans change all the time. It's best that you contact the 800 number on the back of the card to make sure if you have a deductible , has it been meet and does a Sleep study require a prior authorization and or a referral.

​I received a notice that my insurance company has paid on my bill -- but I can't understand how they calculated their payment amount. Do you know?

​Once information from your insurance company is received, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you generally need to check with your insurance company.
